Licensed Practical Nurse (LPN/LVN)

Key Responsibilities • Collect patient histories, vital signs, medication information, symptoms, and other clinical data and communicate significant findings to the supervising clinician.• Administer medications, immunizations, treatments, and routine nursing interventions as authorized by applicable law, organizational policy, and the assigned plan of care.• Prepare patients, examination rooms, instruments, and supplies for clinical visits, tests, and procedures.• Perform approved clinical procedures, specimen collection, point-of-care testing, wound care, and other nursing activities within the licensed scope of practice.• Reinforce provider instructions and educate patients and caregivers about medications, treatments, follow-up care, preventive services, and when to seek additional medical attention.• Document assessments, interventions, patient responses, communications, and care activities accurately and promptly in the electronic health record.• Identify changes in patient condition, safety concerns, abnormal findings, or matters outside established protocols and escalate them to the appropriate licensed clinician.
