San Luis Care Center in San Luis, CO is seeking an LPN Ambulatory Care nurse to provide compassionate, resident-focused care in our skilled nursing and rehab setting. The LPN will perform assessments, administer medications and treatments, monitor vital signs, and support rehabilitation plans under RN and provider direction. You’ll coordinate with an interdisciplinary team, document care accurately, educate residents and families, and help maintain a safe, dignified environment. This role offers close relationships with residents, teamwork, and the chance to grow skills in skilled nursing and rehabilitation.
