We offer reimbursement for continuing education and licensure fees.Competitive Salary of $94,390-$113,152 annuallyWho you are:We are seeking a motivated Diabetes Program Registered Nurse for a Full Time role working in a clinical setting. As a member of the health care team, the Diabetic RN provides quality care to the Diabetic patients of MACT. The Diabetic RN will:Travel to all MACT clinics required on a regular schedule for individual needs assessment of diabetic patients and coordinate diabetic activities between all sites. Provide DSMES services and ADA curriculum across all locations.Assist with planning and developing the Diabetes Program including outcome targets and measures such as A1C reduction rates, retention rates and program completion. Continuously implement quality improvement initiatives for patient engagement and outcomes. Implement and coordinate program activities with other staff, maintaining quality improvement activities that include but are not limited to patient record review, updating tracking systems, maximizing accuracy of data and monitoring compliance with GPRA, GPMS and other diabetes standards as may be required. Oversee program compliance with ADA, CDC, HRSA, HIS and SPDI grant requirements.Prepare and submit required reports for grants, regulatory agencies and leadership. Monitor program budgets, productivity and resource utilization. Identify funding opportunities and assist with grant development and renewals.Develop, implement and maintain ongoing diabetes care activities in order to accomplish established outcome targets. Provide enhanced case management services for diabetic and pre-diabetic patients. Develop and oversee individualized patient care plans. Represent the Diabetes Program at all required meetings, conferences and activities. Participate in community health events and promotes diabetes prevention awareness throughout service area.Update the diabetic work plan annually.Interface with patients and community regarding diabetes care to provide bilateral communication. Conduct patient education classes including CDC Diabetes Prevention Program. Use medical equipment required for diabetic care.Complete medical records forms, statements, summaries, and referrals appropriately, consistently in the Electronic Health Records in a timely fashion.Abide by all company policies and procedures with particular attention to confidentiality and contract health services (CHS).Document in the Medical Record all diabetic care services provided; including telephone counseling and coordinate maintenance of diabetic flow sheets at different clinic sites.Other duties as assigned.Maintain and update training in diabetes.What you need:R.N. licensure with Diabetes Educator Licensure or license eligibility required within two years.Knowledge of community diabetes resources. Demonstrates competency in caring for patients of all age groups, including children, adolescents and elderly.Strong written and verbal communication skills. Strong interpersonal skills as demonstrated through the ability to establish and maintain effective working relationships with other departments within the MACT organization. Experience with automated data systems and EHR, NextGen preferred.Supports an ethical standard, which complies with a code of conduct free of conflicts of interest. Current BLS/ALS certificateMust pass Occupational Health Screening.Must possess a valid form of identification as listed on the U.S. Department of Justice I-9 form, verified using the E Verify system.Clean DMV driving record and MACT auto insurance driving eligibility (determined by auto insurance carrier).The position requires an awareness and keen appreciation of American Indian traditions, customs, and socioeconomic needs and the ability at all times to meet and deal effectively in contacts with Indian health organizations which requires tact, courtesy, discretion, resourcefulness and good judgment in handling functions of a sensitive nature.AMERICAN INDIAN PREFERENCE: Preference in filling of a vacancy will be given to qualified American Indian applicants in accordance with the Preference Act (Title 25, U.S. Code, Sections 472 & 473) and Public Law 93-638, provided the applicant has submitted appropriate verification of Indian preference for employment.
