In this role, you will coordinate and optimize daily visit schedules for nurses, therapists, and other clinicians, ensuring patients receive timely, high-quality care at home. You’ll manage appointment confirmations, handle changes, and maintain accurate information in our EMR and scheduling systems. As a key communication link between patients, families, and field staff, you’ll provide exceptional customer service, resolve scheduling issues, and support compliance with care plans.
