Hospice Physician Designee

Heartland Hospice, part of ProMedica, seeks a Hospice Physician Designee to support medical oversight for patients with life-limiting illnesses. In collaboration with the hospice medical director and interdisciplinary team, you will review and certify terminal diagnoses, develop and update care plans, manage pain and symptoms, and ensure regulatory and quality standards. The role includes family communication, goals-of-care discussions, supervision of advanced practitioners, and coordination with community physicians. Ideal candidates bring strong clinical judgment, hospice or palliative experience, and a deep commitment to compassionate, patient-centered end-of-life care.
