LifeBridge Health, Inc. seeks a Nurse Navigator RN to guide patients through complex care pathways across our regional health system. In this patient-centered role, you will coordinate appointments, education, and resources from diagnosis through treatment and follow-up, removing barriers and improving outcomes. Collaborating with physicians, social workers, and allied health teams, you will perform clinical assessments, monitor symptoms, support care transitions, and advocate for patients and families. You’ll help drive quality, safety, and evidence-based practice while contributing to community health in a compassionate, mission-driven environment with opportunities for learning and growth.
