Geriatric Primary Care Nurse Practitioner – PACE Program

BoldAge PACE seeks a compassionate PCP Nurse Practitioner to provide primary care to older adults in a comprehensive, team-based PACE setting. In this role, you will conduct assessments, manage chronic and acute conditions, coordinate interdisciplinary care plans, and support participants and families in care decisions.
