Client Advocate

Morris Heights Health Center is seeking a compassionate and dedicated Client Advocate / Community Health Worker (CHW) to join our Healthy Start Program.

Key Responsibilities: • Conduct community outreach and engage families in need of support.• Provide case management, client advocacy, and referrals to community resources.• Assist families with accessing Medicaid, WIC, housing, childcare, and healthcare services.• Conduct home visits and provide maternal and child health education.• Maintain accurate client records and coordinate follow-up services.
Qualifications:• Associate degree in Human Services, Social Work, or a related field; OR high school diploma/GED with at least two years of relevant experience.• Experience working with children, families, or underserved communities.• Strong communication, organizational, and interpersonal skills. xzbcwsv • Knowledge of Bronx community resources is preferred.• Ability to communicate in English & Spanish required
